Yapi Leap's Medical History Form
We love all of Leap's new forms, but Leap's medical history is something special. Not only does it integrate medical conditions from the form to patients' Medical History Alerts in Eaglesoft, it also gives you the freedom to format, customize, and add logic to questions more than ever before.
While Yapi Classic required you to build your form in Eaglesoft and import it to Yapi, you can now build and maintain your form right in Leap so everything's all in one place. And when patients submit their forms, those all import to one place too: the patient's SmartDoc.
Note
While Leap forms integrate with Eaglesoft to update medical conditions, medications, and medical alerts, Leap forms do not update the Medical History Questions section of the Eaglesoft Medical History. Instead, this information is available on patients' completed forms in SmartDoc. This allows us to provide you more flexibility in creating and editing your forms.

Any time a patient completes their medical history form through Leap, Leap files a copy in the patient's SmartDoc, which is where you can view their medical history now. Leap still adds the medical conditions they selected to their Alerts, but if you want to see a copy of their form, follow the steps below:
In Eaglesoft, click the SmartDoc icon

Search for the patient and select them, then click Use.
You'll see the patient's documents appear as PDFs in a list on the left, listed by date with the most recent at the top. You can:
Scroll through by date and double-click to open it, or
Click the Documents by Group button at the bottom of the column, double-click the Documents by Group folder, double-click Medical Histories, then find the dated Medical History form and double-click to open it.
Leap integrates your list of Medical History Alerts from Eaglesoft onto your Leap medical history form automatically so whenever you update the list in Eaglesoft, it updates your Leap form. And you can curate your list even further on the form itself, removing any alerts you want to keep in Eaglesoft but don't want on your form.

To add or edit a medical condition on your Leap medical history form follow the steps below.
In Eaglesoft's Practice Management Mode, click Lists, General Setup, then Medical History Alerts.
Click New to add a new alert or double-click and existing alert to edit it.
Note
In some cases, Eaglesoft may not allow you to edit an alert. In this case, we recommend you add a new alert, then remove the old one from your form.
Allow time for your updated Medical History Alerts list from Eaglesoft to automatically sync to your form. (This can take up to 6 hours, so we recommend updating your list before you leave for the day and letting it sync overnight.)
Optional: To make sure your changes appear on your medical history in Leap, preview it in the Form Editor:
In Leap, click Settings, Practice Settings, then Forms Library & Editor.
Use the search box to find you medical history form and click on the form's name to open it..
Scroll down to the Medical Conditions question. This list should reflect your Medical Alerts list you have in Eaglesoft, including the changes you made.
To remove a medical condition so it doesn't appear on your Leap medical history form, follow the steps below.
In Leap, click Settings, Practice Settings, then Forms Library & Editor.
Use the search box to find your medical history form.
If the Modified by column of your form reads "Yapi," create a copy of the form that you can edit. If this column reads anything else, your form is already editable - just go to the next step!
Click on the form's name to open it. Then click Edit to open it in the Form Editor.
Scroll down and click on the Medical Conditions question. This list reflects your Medical History Alerts list in Eaglesoft. For any condition you don't want to appear on your form, slide its toggle to the left (disabled).
Note
These changes don't affect your list of Medical History Alerts in Eaglesoft. Disabling a medical condition here just keeps it from being shown on your form.
Click away from the question so that Leap automatically saves your form.
Click Preview, then Activate to reactivate the form for patient use.
