Assigning or Reassigning a Form to a Provider

For forms that require a provider signature before filing, you can assign each form to the provider that needs to sign it, making it easier for them to find. Just follow the steps below.

  1. Click Forms in the menu on the left.

  2. Select the Outstanding Tasks tab.

  3. Find the form you'd like to assign to a provider.

    • Optional: Use the Tasks filter to narrow down the list of forms that display.

  4. At the end of the row, click the 3-dot icon 3-dot icon. and select Assign or Reassign:

    • Assign - Select this option if the form is listed as Unassigned

    • Reassign - Select this option if the form has an assigned provider and you need to assign it to different one.

  5. In the pop-up that opens, select the provider you want to assign (or reassign) the form to.

  6. Click Confirm. You'll see a message at the bottom of the page confirming your change.
