Resending Forms
Leap makes it easy when you need to resend forms to a patient (like when they forget or just can't find the link they were sent). Click the links below to learn how.
On occasion, you may find you need to resend forms to your patients before their appointment. Maybe they lost their message or maybe they're a new patient who needs to get their forms via email instead of in a New Patient Welcome text.
And every once in awhile, you may also have a patient trying to access forms from an expired link. Links can expire because:
the patient's appointment was cancelled.
the patient's appointment has passed or they were checked out of Leap.
the patient failed to verify their identity by entering the correct date of birth when trying to access their forms.
Whatever the reason, if you need to resend forms, just follow the steps below!
Click Forms in the menu on the left.
Click the Pending Forms tab.
Scroll down to look for your patient's forms or use the search bar to find the patient by name.
If your patient didn't report any problems: Skip to the next step.
If your patient did report a problem:
...and you find your patient's forms listed: This simply means their link expired because they couldn't verify their identity when they tried to access their forms. Double-check with the patient that their birthdate is correct in Eaglesoft, then go to the next step.
...and you don't find your patient's forms here: This means they were trying to access forms from a previous or cancelled appointment or that they've been checked out in Leap from their current appointment. Go directly to creating a new forms request for the patient by following the steps in Assigning Forms Before a Patient's Appointment.
Click the 3-dot menu
for the patient (on the right) and select Resend requested form.
Select where you'd like to resend the form(s):
Text - Sends a text to the patient with a unique link to their form(s) for them to complete.
Email - Sends an email to the patient with a unique link to their form(s) for them to complete.
Text and Email - Sends a text and an email to the patient with a unique link to their form(s) for them to complete.
Click Resend. You'll see a confirmation message at the bottom of the page confirming the forms have been resent. Patients can use the new link you've send then to access their forms or use the original link they were sent.
Note
When accessing forms through a previously sent link, the patient must use the link sent for these forms. Links sent for other forms or previous appointments won't open.
If your patient has arrived for their appointment and didn't get a chance to fill out the paperwork they were sent, no problem! Just use the instructions below to resend their forms.
Click Forms from the menu on the left.
Click the In Office Patients tab.
Find the patient you want to resend forms to and click the 3-dot menu
(on the right).
Select Assign incomplete forms to device.
Select where you'd like to resend the form(s):
Linked Device - Sends the form(s) to one of your practice's already-linked devices (like a tablet or kiosk) for the patient to complete in office.
QR Code - Sends the form(s) to any practice tablet, once you use that tablet to scan the QR Code you'll be given.
Text - Sends a text to the patient with a unique link to their form(s) for them to complete. With this option, they must verify their identity for security purposes to access the form/package.
Email - Sends an email to the patient with a link to their form(s) for them to complete. With this option, they must verify their identity for security purposes to access the form/package.
Review the list of incomplete forms currently assigned to the patient.
If you want to assign additional forms to the patient:
Click +Form, choose Packages or Forms, and select a package or form that the patient needs to complete. Repeat for each package and/or form you want to assign to the patient.
Optional: If you assigned more than one form or package, you'll see a Custom Package Name field appear, since this is a custom package you're creating for your patient. Enter a name for the package (like "New Patient Intake & Consent Forms").
If multiple family members are checked in, you'll see their names appear in tabs when selecting forms. Assign forms to each other family member by clicking on their tab and repeating Steps 5-7.
If you don't need to assign additional forms, proceed to the next step.
Click Send. You'll see a confirmation message at the bottom of the page.
If you chose QR Code in step 5, a window displays with a one-time use QR Code. Grab your office's device and scan the QR Code. You can then hand the device over to the patient.
