Using Yapi’s Consolidated Dental History & Medical History Forms

With the 3/12/25 release, Yapi has created new consolidated Medical and Dental History forms which now shows different questions based on a patient's age and can be used for both adults and children. In order to use these forms, the you'll need to review and update some Leap settings. Select a link below to continue.

Note

If your office has form customizations and you're not interested in using Yapi's updated Dental and Medical History forms, no action is required. You can continue using the customized forms you already have!

Since your forms don't have any customizations, getting the updated medical history and dental history forms is easy! Just follow the instructions below to get your forms and update your Leap settings.

Step 1: Grab the Forms
  1. Use these instructions to get the updated medical history and dental history from the Form Templates page. These new forms are called "Med History - Eaglesoft" and "Dental History".

  2. Optional: If the legal age of a minor is over 18 in your state (for example, Alabama, Nebraska, or Mississippi), update the form you just brought over to edit the question and update change the age to the appropriate age for your state.

    1. Click the blue View Duplicated Template link at the top of the Forms tab on the Templates library.

    2. Open the form "Med History - Eaglesoft" or "Dental History form"

    3. Click Edit to open the form.

    4. Click on the "Is the patient 18 or older?" question.

    5. Change the 18 to whatever number you'd like. You can even rephrase the question. Just make sure change it too much since the logic rules are intended to tell whether the patient is an adult or child.

  3. If your old Medical and Dental History forms were automated, set automation on each of the new ones too.

  4. Keep your existing medical and dental history forms active for a little while, just in case any patients are in the middle of completing them. Patients who already started the existing forms won't get the new ones automatically.

  5. Optional Medical History Spanish Translation: If you want your Medical History form available in Spanish, you'll need to add translations for your practice's list of medical conditions and allergies:

    1. Refer to our Spanish Translations for Medical Conditions & Allergies article to see a list of common medical conditions & allergies that have already been translated for you.

    2. Now open the form from your Form Library to add the translations:

      1. Click the Translations Translations icon. button (it probably says "English" right now) and select the language you want to provide a translation for. Your form now shows placeholders for all the questions you've already added in English.

      2. Click on the Medical Condition or Allergies question to edit the question.

      3. For each medical condition or allergy, copy the translation from the translation article and paste the translated medical condition or allergy into the field. Each time you click away from a question, Leap will automatically save it and you'll see a message confirming this at the bottom of the page.

      4. Repeat steps ii and iii for each medical condition and allergy that you need to add Spanish translations for.

Step 2: Update Your Packages

Review any existing packages that include the old forms. You'll either want to replace these old forms in your packages or create new packages altogether. The old forms are called:

  • Med History (Adult) - Eaglesoft,

  • Med History (Child) - Eaglesoft,

  • Dental History (Adult)

  • Dental History (Child)

  • Dental History (Child) - Short

To update an existing package with the new forms:

  1. Remove the old forms from your new patient package(s) using these instructions.

  2. Add the new Med History - Eaglesoft and Dental History forms using these instructions.

To create a new replacement package using the new forms:

  1. Create a package with with the new forms using these instructions.

  2. To set automation for the new package, use these instructions.

  3. If your new package replaces a couple old ones (good for you for consolidating!), get those cleaned up by deleting your old packages.

Step 3: Update Your New Patient Text Appointment Reminder

One more thing! Let's make sure your new forms and packages are sent with your appointment reminders:

  1. In the left-hand menu, click Settings, then Practice Settings, and select Patient Reminders.

  2. New Patient Welcome - Text Message:

    1. If you just updated your existing packages in the previous step, you're all done! (Nice job 😁)

    2. If you created a new package, use these instructions starting at Step 3 to update your New Patient Welcome Text Message with the new package.

Some Things to Keep in Mind

So what happens if a patient already got the old forms? And when do patients start getting the new ones?

  • Patients who got the old forms and started them already: These patients won't get the updated versions this time. They'll keep the old versions so they don't lose their progress and get the updated versions next time they're due to fill them out.

  • Patients who got the old forms and haven't started them yet: They'll get the new versions automatically! When they log in to complete their forms, Leap will have already replaced the old ones with the new.

  • Patients who haven't been sent the old forms: Clean slate! All these patients get the new forms moving forward.

Use the instructions below to get your forms and update your Leap settings.

Step 1: Update your Forms
  1. Use these instructions to get the updated medical history and dental history from the Form Templates page. These new forms are called "Med History - Eaglesoft" and "Dental History".

  2. Now compare your customized form with the updated "Med History - Eaglesoft" and "Dental History" forms and make note of any questions you added to your customized form that you want to continue using.

  3. Open Yapi's updated "Med History - Eaglesoft" or "Dental History" form that you brought over from the Templates library in step 1.

  4. Add your customized questions to the form. Just drag and drop questions onto your form from the right panel based on the questions you made note of in step 2. Repeat this step for each question you'd like to add to the form.

  5. Optional Medical History Spanish Translation: If you want your Medical History form available in Spanish, you'll need to add translations for your practice's list of medical conditions and allergies:

    1. Refer to our Spanish Translations for Medical Conditions & Allergies article to see a list of common medical conditions & allergies that have already been translated for you.

    2. Now add the translations to your form:

  6. Preview and activate the form once you're done customizing it.
