Forms Templates
Yapi Leap provides dozens of pre-made forms templates you can use so you can start sending forms to patients right away! This in-app lists allows you to browse our full offering and send the forms you want to your library any time you want. And if you've made edits to a pre-made form but want to start over, there's a clean copy always waiting here for you.
Important
While these are standard forms used across the dental industry, you should always consult an attorney before using any forms with patients.
Parts of the Forms Tab on the Templates Page
To get to the Forms tab of the Templates page, click Settings > Practice Settings > Templates. The Templates page defaults to the Forms tab.

Only users with Admin privileges can access this page.
