Moving Custom Forms from Yapi Classic to Yapi Leap
If you previously used Yapi Classic (your desktop Dashboard) for patient forms, you might have some custom forms you'd like to move over to Leap. With the new Leap Form Editor, it's easy! Just follow the steps below to copy over each of your custom forms.
Important
Only users with Admin privileges can perform this task.
On an office iPad, use the Yapi iPad app (our Classic version) to view the custom form
you'd like to recreate in Leap.
In Leap, click Settings in the left-hand menu. Then click Practice Settings and select Forms Library & Editor.
Click New Form and select Create from Scratch.
In the Form Editor, type in the Form Name.
Select a Form Type and the Eaglesoft Category you want the form to file to when patients complete it.
Click the Click to add header title box, then click Add header title to type in the title of your form.
Optional: Click Upload image to add your practice's logo.
Click the Drag and drop fields here box.
Use the form on your iPad as a reference to add questions and other elements to the form like blocks of text and signature boxes. Every time you click away from a question, Yapi automatically saves your changes.
Note
Except for Patient Information forms, you can skip adding text boxes for the patient to fill in their name. Leap adds the patient's name to the form automatically! But for Patient Information forms, make sure to use a Patient Info question. This lets patients update their name if they need to.
Tip
Don't want to type it all in? You can copy the text from your form in Yapi Classic by navigating in your Dashboard to Setup and Manage > Manage Documents and Templates > iPad Forms. Then select the form you're working on, click Edit, and when the Form Builder opens, double-click the question you want to copy. Then highlight and copy the text.

Once you've added all your questions, click the Preview button.
Click Activate so your team can now give the form to patients.
