Yapi Leap's Forms feature helps you easily manage paperless forms so patients have a seamless experience and you save some trees in the process! 🌳 Here are some things you can do with Leap forms:
Templates & Customization: Use pre-made templates to get started right away or customize to your heart's content in the Form Editor.
Packages: Set up packages for forms that go together often like new patient intake paperwork.
Multi-Language: Provide forms to your patients in 3 different languages. (Our pre-made templates are already available in English and Spanish!)
Email, Text, or Use a Device: Send forms via email or text at any time or have patients use an in-office device (like a tablet) when they arrive for their appointment. And whenever you send them, whether it's ahead of time or at the appointment, returning patients don't need to fill out questions that integrate with Eaglesoft- they can just review and update! This includes questions on the their patient information and medical history forms and any other questions that are tagged to integrate with Eaglesoft.
Track Progress: View patient progress from the Forms page to see what's done and what still needs attention.
Set Automation: You can also set automation for individual forms and packages so Leap automatically sends patients the forms they need with their appointment reminders. No need to assign each form to each patient!
Send to Families All at Once: You can even send forms to families as a unit so they can fill them out together. (And forms that have automation set do this automatically.)
