Deleting a Patient’s Form

While it's not very common, you may find you need to delete a patient's completed form because they filled out the wrong one or because they entered the wrong information and need to fill out the form again. If you need to delete a form from Yapi Leap (completed or not) click the appropriate topic below to learn more.

Note

This only applies to forms that haven't already been automatically filed into Eaglesoft. If Leap has already auto-filed a form into Eaglesoft, you'll need to delete it there instead.

Tip

For patients that don't show up for their appointments or don't need to complete a form you assigned to them, we've got you! Leap automatically removes all forms that weren't completed by a patient when you check them out or at midnight on the day of their appointment, whichever comes first.

Use the instructions below to delete a form for a patient with a future appointment.

  1. Click Forms in the menu on the left.

  2. On the Pending Forms tab, find the patient who has the form you'd like to delete, either by scrolling down or searching for the patient using the search bar.

  3. Use the down arrow Down arrow icon. to expand and see the list of forms assigned to the patient.

  4. On the form that you'd like to delete, click the 3-dot icon 3-dot icon. at the end of the row and select Delete.

  5. At the Delete Form? prompt, select Yes, Delete.

Use the instructions below to delete a form for a patient that's currently checked in for the appointment.

  1. Click Forms in the menu on the left.

  2. Click the In Office Patients tab.

  3. Scroll down to find the patient with the form you'd like to delete.

  4. Use the down arrow Down arrow icon. to expand and see the list of forms assigned to the patient.

  5. On the form that you'd like to delete, click the 3-dot icon 3-dot icon. at the end of the row and select Delete.

  6. At the Delete Form? prompt, select Yes, Delete.

Use the instructions below to delete a form for a patient that has already been checked out.

  1. Click Forms in the menu on the left.

  2. Select the Outstanding Tasks tab.

  3. Find the patient with the form you'd like to delete.

    1. Optional: Use the Tasks or Provider filter to narrow down the list of forms that display on the screen.

  4. Click the 3-dot icon 3-dot icon. at the end of the row and select Delete.

  5. At the Delete Form? prompt, select Yes, Delete. You'll see a message at the bottom of the page confirming the form was deleted.
