Sending a Leap Template to Your Forms Library
Yapi Leap has plenty of pre-made forms templates to choose from so you don't have to reinvent the wheel! You can send any pre-made forms you don't have to your Forms Library to use right away or grab a clean copy of a form you've edited if you want to start over. Just follow the steps below.
Important
While these are standard forms used across the dental industry, you should always consult an attorney before using any forms with patients.
Click Settings from the menu on the left and select Practice Settings.
Click Forms Library and Editor.
Click the Templates button.
Find the form you want, click the
3-dot icon next to it, and select Duplicate. You'll then see a message at the bottom of your screen confirming the form has been duplicated. You'll also see a green banner letting you know you need to return to the Forms Library & Editor to use the form.

Can't find a form that you saw in Forms Templates before? Contact Yapi Support so we can take a look.
Click the View Duplicated Template link in the green banner to return to the Forms Library & Editor.
Find your form in the list. (It'll be called "Copy of" plus the form name.) Then click the 3-dot icon
for the form and select Edit.
Under Eaglesoft Import Category, select the category that this form best fits in.
Optional: Make any edits to the form that you like.
Click Go Back to Forms Library and Editor.
Click the 3-dot icon
for the form and select Activate, then click OK. Your form is now available to use with patients. (Woohoo! 🎉)
