Drag-and-Drop Questions in the Form Editor
When you're editing or creating a form in Yapi Leap, you'll see a list of question types and formatting elements (like grids and groups) that you can drag and drop into your form. Below, we've provided the basics about how each one works but we recommend you experiment with the form editor yourself to learn even more!

Contact Info | Adds spaces for patients to provide their home, work, and mobile phone numbers, email address, and street address. This question type is data-integrated which means it pulls information from Eaglesoft onto a patient's form and updates that information in Eaglesoft when Leap files it. Data-integrated questions have a gold arrow icon The parts of this question can't be modified or reordered except for the State and ZIP. You can change the names of these (for example, to Province and Postal Code if you're in Canada) by double-clicking on them and typing in a new name. | |
Text Box | Allows you to ask a question that needs a written response (in other words, a short answer or long answer question). You might use this to ask if a patient has concerns about visiting the dentist. | |
Multiple Choice | Allows you to ask a multiple-choice question with answers that you set. By default, patients have to choose a single answer but you can toggle on Checkboxes if you want, letting patients select multiple answers. | |
Dropdown | Allows you to ask a question where patients can select their answer from a dropdown menu. | |
Yes/No | Adds a question where the answer is a simple yes or no. NoteFor medical conditions, allergies, and medications, don't use this question. Use the separate Medical conditions, Allergies, and Medications questions instead and in each question, toggle Yes/No Input to on (orange). | |
Range | Allows you to ask patients for a range of numbers, dates, or times as an answer. For example, you might use this to ask a patient for the start and end dates of their insurance coverage or for their general appointment availability. | |
Matrix | Allows you to ask patients to provide answers about several items on a scale. For example, you might ask patients how often they brush their teeth, floss, and use mouthwash with the answers "Never," "Sometimes," and "Always." You can also toggle on Rate Mode to use star, thumbs up, or heart icons for answers options instead of the standard round buttons. | |
Medical Conditions | Allows patients to select any medical conditions they have. You can toggle on Yes/No Input to have each medical condition display with yes and no options. This question type is data-integrated which means it pulls information from Eaglesoft onto a patient's form and updates that information in Eaglesoft when Leap files it. Data-integrated questions have a gold arrow icon ImportantThe list of conditions a patient can choose from reflects the list you've set up in Eaglesoft. If you need to make changes to conditions on this list, do it in Eaglesoft and the changes will be reflected on your form. NoteIf you remove individual medical conditions from your Leap form or from your lists in Eaglesoft, they won't appear on future versions of the form, even if a patient selected them on a previous version. | |
Smart Signature | Adds a signature field to the form and you can choose the kind of signature it needs to be: Patient/Parent/Guardian or Provider. If you choose Patient/Parent/Guardian, this adds a space for them to sign and a dropdown for them to select their relationship to the patient. If you choose Provider, this adds a space for the provider to sign and a dropdown of your practice's providers so the provider can select their name. Provider signatures spaces also don't appear when patients are completing their paperwork so they don't accidentally sign where they're not supposed to! NoteEach Leap form can have up to 4 signatures total (this includes Smart Signatures and Standard Signatures together) and only 1 can be a Smart Signature for a Provider. We recommend adding a signature field at the bottom of every form you have. | |
Standard Signature | Adds a simple signature field to the form. Unlike Smart Signatures, this doesn't include a relationship to patient or provider name dropdown list. NoteEach Leap form can have up to 4 signatures total (this includes Smart Signatures and Standard Signatures together). | |
Picture | Allows you to add a picture onto the body of your form like an example of a procedure or your practice's logo. You can use .jpeg, .png, or .gif files. We recommend using an image 100 to 800px wide and 50 to 500px high. | |
Upload File | Provides a space on the form for patients to upload a file like their ID or insurance card. (Leap accepts PNG, JPG, JPEG, GIF file formats.) When the form imports into Eaglesoft, the form and the uploads are saved as separate files, so you can have them import to different folders if you like. If the patient uploads a file, you'll be able to to see a preview of the file on the Pending Forms tab and be able to click on it to to view the full image in a new Leap tab. Leap automatically sends the uploaded files to the same location you set for the the form to go in the form's Eaglesoft Import Category field. You can change this location to a different destination in Eaglesoft if needed. | |
Rate | Provides a rating scale where patients can rate with stars, thumbs up icons, or hearts. This is a great option for gathering feedback about a previous visit. | |
Plain Text | Allows you to add text, like instructions or long paragraphs of information. This is often used for consent form to explain the details of a procedure and list the risks. TipTo add a link within the text, highlight the text you want to add your link to, press Ctrl + K (Windows) or Cmd + K (Mac) on your keyboard, type or paste in the link address, and click Save. To start a bulleted list, type an asterisk (*) on a new line and hit Enter on your keyboard. | |
Group | A group itself isn't a question type; it's a way for you to group questions together. Just drag a Group onto your form and add questions into it. TipWhy use a group? Leap forms allow for Logic Rules, which tell a form to show or hide a question - or an entire Group of questions - depending on a patient's answer to a previous question. So, for example, you could make an insurance form that hides all the questions asking for insurance information when a patient says they don't have insurance. | |
Grid | A grid isn't a question either! It just allows you to create a grid of questions so you can set them next to each other and save space. Just drag a Grid onto your form and add questions into it. |
