Viewing a Patient's Completed Medical History Form in Eaglesoft

This article only applies to offices using Leap's Medical History form. Offices using the integrated Eaglesoft Medical History form can just go directly into Eaglesoft to see patient responses.

Any time a patient completes their medical history form through Leap, Leap files a copy in the patient's SmartDoc, which is where you can view their medical history now. Leap still adds the medical conditions they selected to their Alerts, but if you want to see a copy of their form, follow the steps below:

  1. In Eaglesoft, click the SmartDoc icon Eaglesoft SmartDoc icon..

  2. Search for the patient and select them, then click Use.

  3. You'll see the patient's documents appear as PDFs in a list on the left, listed by date with the most recent at the top. You can:

    • Scroll through by date and double-click to open it, or

    • Click the Documents by Group button at the bottom of the column, double-click the Documents by Group folder, double-click Medical Histories, then find the dated Medical History form and double-click to open it.
