Deciding Which Medical History Form to Use
Having options is great and with Leap you can decide whether to use Leap's customizable Medical History form or the Medical History form from Eaglesoft. Review the information below In order to decide with option is best for your practice.
It's simple: The Leap Medical History form gives your more flexibility. It's a great option for offices that need to gather more information than what's offered in Eaglesoft's Medical History form or to display questions in a more user-friendly way. This is a great fit for offices that want to:
Add a practice logo to their form
Add customizations to the form
Add questions that the standard Eaglesoft Medical History form doesn't have
Make important questions on the form required
Use formatting or logic rules to display questions in a patient-friendly format and easily mark questions required
Use Leap's Smart Signature field to capture the relationship of the person signing (patient/parent/guardian or provider)
Using Leap's File Upload component makes it easy to upload a patient's copy of their medications list
Integrate Medical Alerts into the patient's file while giving you more capabilities than Eaglesoft's standard Medical History form
One form can be translated and offered in multiple languages
The form can be edited even if patients have already submitted responses

Using the integrated Eaglesoft Medical History form is a good option for offices that:
need patient responses to display in the Eaglesoft Medical History Module
want a 1:1 integration with the matching Medical History form in Eaglesoft
find that the most important feature of the Medical History form is the ability to update the Eaglesoft Medical History Module
have a simple Medical History form that doesn't require additional special questions or formatting that can't be created in Eaglesoft
want to prioritize:
having limited form customizations in Eaglesoft
NOT using required fields (the Eaglesoft Medical History form doesn't have this option)
maintaining multiple Medical History forms (adult/child, English/Spanish)
starting fresh with a new form when edits are needed (Eaglesoft doesn't allow edits to a form once any patient response has been received. You'll need to duplicate the form to perform an edit, which resets prefilled responses.)
Some things you should know about this option:
Updates made in Eaglesoft sync in Leap every 5 minutes.
Any previous information a patient completed in their Medical History will be pre-populated on this form.
Once a patient completes their form, Leap updates Patient Responses in the Eaglesoft Medical History Form and files the form to SmartDocs
